- Explaining health-care resources and policies to patients, family and professional staff
- Helping patients and families receive needed follow-up care by referral to health care resources
- Explaining to patients the environmental risk factors associated with their disease process
- Assisting with the planning for post-hospital patient needs, by arranging for services at another facility or in the home
- Understanding the social, cultural and religious variables that contribute to patients’ responses to illness and their use of health-care resources
- Providing advocacy through appropriate organizations
- Planning and delivery of care
- Referral of new patients
- Evaluation of results for each patient and adjustment of the care plan
- Evaluation of health program effectiveness & adjustment as neccesary
